If a payor expects a treatment plan, and there isn’t one in the chart, the requirement still applies. And it negatively impacts the review.

Circle now functions similarly. When a review searches for a required document and finds none, the question fails instead of being marked Not Applicable.

This means your score may change. That’s the point. Requirements that were previously outside the calculation now appear alongside all the other items your team needs to close.

The result is a compliance score that more accurately reflects the standard your chart will be evaluated against during an audit.

It’s important to inform your team about this change before the monthly report is released.

Every review indicates the currency of its data

Circle reviews a synchronized copy of your chart. Now, every chart review displays the latest EHR sync date alongside the result, allowing you to instantly assess the timeliness of the underlying record.

For instance, if a clinician reports that a missing note was added this morning, you can swiftly determine whether Circle has already encountered it. This distinction helps differentiate between a documentation gap and a chart that simply hasn’t been synced.
